Four Decision Types
A reviewer chooses one of four outcomes: retire, resolve, ignore or reject. The reason is required rather than optional, so the record answers why as well as what. Six months later the question is rarely whether a decision was made, but what the reasoning was, and that is the part most systems fail to keep.
